Before our first meeting, it is important that you print and complete all of the forms below. These forms are for our office records, and will be kept in a safe place. All the information given on these forms will remain confidential. Please also remember to bring your insurance card with you on our first meeting. Should your insurance info change at any time during your treatment, you need to print and complete a new billing information form for our records. If your insurance denies payment, you will be responsible for those charges.
New Patient Packet
This packet is for new patients. Please print it and completed it. Remember to bring this form with you on your first visit so that we may begin our work as quickly as possible.
Client Billing Information Form
Please complete and bring this from to your first visit. It is your responsibility to give us an updated version of this form every time there is a change in your personal or insurance information during our time together. If we do not have the correct billing information resulting in a denial of payment, you will be responsible the amount outstanding.
HIPPA Form
Please print a copy of these forms, sign and date them and bring them with you to our first visit. Keep a copy of these forms for you records. Please remember to bring these forms with you so that we may begin our work as quickly as possible.
